How to open a therapy or rehabilitation clinic in the UAE

Opening a therapy clinic in the UAE means licensing two things: the facility and every professional in it. In Dubai the facility licence runs through the DHA and requires approved premises, a medical director, a compliant EMR and sign-off from several authorities. Both the facility and the individual licences must be in place before you see a single patient.

Two licences, not one

The most common misunderstanding among first-time clinic owners is thinking a single licence covers everything. It does not.

The facility licence is issued to the clinic as a business and premises.
A professional licence is required for every therapist, doctor and allied-health professional who works there.

Both must be in place before the clinic treats anyone. A licensed facility with unlicensed staff, or licensed staff in an unlicensed facility, cannot legally see patients.

What the facility licence requires (Dubai / DHA)

For a Dubai clinic through the DHA, the facility side typically involves:

Premises paperwork — the Makani number, Dubai Municipality plot certificate and Ejari tenancy contract.
An approved layout — drawn by a DHA-prequalified design consultant for the specific services you intend to offer, with space standards to meet.
A trade licence and an appointed Medical Director, a post that cannot sit vacant beyond a short window (commonly two weeks).
A compliant EMR meeting NABIDH standards, including the required data set and tested connectivity.

Rehabilitation-specific facility types exist, for example physiotherapy and occupational-therapy centres, each with their own space and equipment expectations. Choose the facility category that matches the services you actually plan to deliver.

The multiple approvals

A clinic fit-out touches several authorities, and they run in parallel rather than one after another. In Dubai you are typically dealing with:

Dubai Municipality (premises and plot).
Civil Defence (fire and safety).
The Dubai Health Authority (health facility approval).
The Department of Economy and Tourism / DED (trade licence).

Sequencing these well is most of the project-management challenge. Delays usually come from premises approvals and the layout sign-off, not from the health application itself, so start those early.

Staffing the clinic

Every clinical professional needs their own active licence with the same authority, verified through DataFlow and, where required, having passed the Prometric exam. When you hire, you can check a professional's status, and patients or partners can check yours, on the DHA facility search.

Plan hiring around licensing lead times. A therapist who still has DataFlow and an exam ahead of them may be months from being able to bill, so a mix of already-licensed hires and in-progress candidates is usually the realistic approach for a new clinic.

Decide services and facility category, and secure suitable premises.
Engage a DHA-prequalified consultant for the layout and begin premises approvals.
Set up the trade licence and appoint your Medical Director.
Implement a NABIDH-compliant EMR.
License your clinical team, prioritising already-licensed hires for launch.
Complete facility approval, then open.

Costs vary widely by size, location and services and change over time, so treat any single figure with caution and budget from current DHA guidance and your consultant, not from a blog number.
